I am newbie friendly this, however, does not mean you do not have to screen.  You will be screened more thoroughly and I require newbies (as in you have zero provider references) to have a 90-minute session for our first meeting. All your information is kept private and secure and after my screening is completed it is discarded. While this is completely safe I do understand a feeling of caution, if you'd rather be screened without filling out this form, I will require the same information but can accept it through a phone call or email. 

Once you have been vetted by me and we've had a fantastic time I'll be happy to give you a reference to any lady that takes them from me for up to 6 months after our meeting. After that, I'll require another appointment to put you back on my referral list.
